Outreach Worker - Rough Sleeper Initiative

We're looking for someone to work as part of a small team on a street outreach service working with new and current rough sleepers.

Job description:

As part of a small team working across the Somerset district of Sedgemoor (Bridgwater, Highbridge, Burnham on Sea and Cheddar) you will:

· Operate a street outreach service working with new and current rough sleepers and linking them to available emergency and longer-term accommodation and support agencies

· Provide assertive resettlement support to house former rough sleepers to enable them to access appropriate longer-term accommodation

· Support the relevant district councils in improving the accessibility of homeless prevention services to single people at risk of rough sleeping

· Work closely with colleagues, volunteers, partner agencies and the local community to ensure rough sleepers receive the support they require and to reduce the level of rough sleeping.
