Dependency Navigator

We're looking for someone to work with clients who require intervention and/or support around high risk drug and alcohol use.


Job description:

As part of a small team working across the Somerset district of Sedgemoor (Bridgwater, Highbridge, Burnham on Sea and Cheddar) you will:

· Work with clients who require intervention and/or support around high risk drug and alcohol use, dependent drug and alcohol use or increasing risk from drug and alcohol use.

· Work as part of a multidisciplinary team, to meet clients’ needs and to provide appropriate support, guidance, and access to treatment.

· Work to reduce the harm caused by drug and alcohol misuse amongst single homeless people, to enable clients to minimise the harm caused to themselves by that misuse and, where possible, to assist them in becoming abstinent and lead more stable and healthy lives. The service aims to improve the health and wellbeing of clients and to support them in taking on meaningful and satisfying social roles.

· Work in partnership with key agencies and take lead responsibility for case management. To carry out a range of drug and alcohol related interventions including strength-based assessment, risk assessment and management, recovery planning, structured group and 1:1 work, brief interventions and motivational interviewing.

Salary:

£19,531.00 to £23,525.00 per year. This is a full time fixed-term 12 months contract.
